Privacy Policy
This page explains how NutriForms (the operator of nutriforms.club) processes personal data that you submit on the Alvarix order request form. Last updated: 19 September 2026.
Controller
NutriForms, website https://nutriforms.club/. For privacy requests, use the same phone number you submitted: the operator who confirms the order can record a correction or deletion request.
Data we process
- name and telephone number;
- country of delivery;
- IP address, browser technical data, referring page;
- advertising identifiers that the browser sends with the form (for example Google Click Identifier), used only to measure whether a request followed an ad.
Purposes and legal bases
We process this data to (1) record your request, (2) call you to confirm or decline the order, (3) arrange delivery if you confirm, (4) prevent fraud, and (5) measure advertising performance. Legal bases under GDPR: taking steps at your request before a contract (Art. 6(1)(b)), your consent given on the form (Art. 6(1)(a)), and legitimate interest in measuring ads and preventing abuse (Art. 6(1)(f)).
Who receives the data
Fulfilment, courier and call-centre partners who confirm and deliver the product, and technical processors needed to host the form and register the request. We do not sell your phone number.
Retention
We keep request records for as long as needed to complete the order, meet accounting duties and handle disputes, then delete or anonymise them. Advertising identifiers are not used for unrelated profiling.
Your rights
You may request access, rectification, erasure, restriction, objection, or data portability, and you may withdraw consent for further calls. You can also lodge a complaint with the data protection authority in Latvia (DVI) or Lithuania (VDAT), depending on your country.
Cookies
The order page may store a first-party cookie for click measurement (including gclid when you arrive from Google Ads) for up to 30 days so the confirmation form can be attributed to the same visit.
